One of the main reasons for the missing link between the services is that historically national health and social care systems have developed in an incremental way, without a coherent vision of how both social care and healthcare systems should interact (Leichsenring, Billings et al. 2014).

SmartCare aims to improve this situation by delivering integrated eCare services in the community, joining in a better way health, social and informal care service provision thanks to the support of information and communication technology (ICT); this is also expected to enhance self-care and empowerment of care recipients and their relatives.
