The Connecare is a novel integrated care model for complex chronic care management. This new model connects care professionals with chronic patients and their care workers with the support of smart ICT solutions. Connecare provides decision support for the adaptive management of personalised clinical pathways and provides smart tools to monitor patient’s activities and status, and providing them with recommendations to self-manage their condition and ultimately improve their quality of life.
