As of 1 April 2023, Form Tools will be decommissioned.

Please note the following:

• it will no longer be possible to collect any submissions via the forms built through Form Tools
• all of the existing forms will be disabled 
• the existing data is accessible and can be extracted until 14 April 2023

The final decommissioning of the tool will occur towards the end of April.
