The project "Integration of people with mental disturbances and
psychiatric illnesses in the labour market" conforms to the priority of
the European Commission (EC) EQUAL initiative to facilitate
accessibility and opportunities of returning to the labour market for
people who find it is hard to join or re-integrate themselves in the
labour market, which shall be open for everybody.
The people with mental disturbances and psychiatric illnesses experience
considerable problems with integration or returning to the labour
market. This fact is evidenced by similar results acquired in various
studies. Only 20% of hospitalised patients in psychiatric hospitals are
employed, 13.7% of them are looking for a job but cannot find one (M.
Caune, M. Taube, 2004), 14.6% of the total number of patients – 60,924 –
work on irregular basis, 26.3% are supported by other people (M. Taube,
V. Lace, 2003). The possibilities of integration in the labour market
for the target audience of the project are related to the progress of
their illness and specific approach is necessary.
The aim of the project is to create and test new support mechanisms that
would allow the target audience of the project to integrate themselves
in the labour market and expand the possibilities of these people to
increase their competitiveness in the labour market. The main directions
of project activities are: studies of the situation in psychiatric
health care facilities in relation to the working skills of the patients
and the actual application of these skills; exchange of experience with
foreign partners; drafting of action programme for definite activities
that have to be taken in psychiatric health care facilities that would
improve the working skills of the target audience and facilitate the
return of these people in the labour market; development of new
political initiatives in relation to psychiatric health care facilities
and their activities (medical treatment, early rehabilitation, social
care, training courses of working skills, provision of jobs); proposals
of amendments to the legislation (principles of funding, compulsory
requirements for medical facilities, etc.), adaptation of infrastructure
for the purpose of training of working skills; provision of jobs in
psychiatric health care facilities.
The project involves psychiatric health care facilities whose day-to-day
operations include treatment of mentally ill people and seeking of
solutions to daily problems that these people encounter. The expected
result of the project is improvement of the legislation that would allow
establishment of new treatment and assistance types in psychiatry
(medical treatment, social care, employment), improvement of working
skills among people with mental disorders, acquisition of experience,
detailed assessment of the situation. The target audience of the project
is persons with mental disturbances and psychiatric illnesses.

The main innovative element of the project is the development of a new
approach to solution of questions related to employment of people with
mental disturbances and psychiatric illnesses with an aim of improving
the possibilities of this group of people in the field of employment. By
now there has been no definite approach as to what would be the
conditions (outpatient or stationary), amount and duration of work for
these people. The innovative element of this project is provision of
training of working skills, assessment and provision of work for people
with mental disturbances and psychiatric illnesses in psychiatric health
care facilities considering the status of their health, the progress of
their illness and the practical connection between the education and
working experience of those people.
Up to this date, there has been no political or legal base for
employment of patients in relation to outpatient or stationary
psychiatric health care service; the approaches used varied from region
to region and from institution to institution. Already the first stage
of the project envisages initiation of assessment of the situation in
psychiatric health care institutions, while the second stage of the
project envisages detailed studies of the situation, adaptation of the
infrastructure necessary for the target audience to implementation of
the activities, provision of work and remuneration for the target
audience at psychiatric health care facilities, development of methods,
political initiatives and proposals of amendments to the legislation,
exchange of experience and assessment by foreign cooperation partners.
In the second stage of project, the studies will address the efficiency
of use of the new service approach towards the target audience along
with corrections in the project activities.
Another innovative element proposed in this project is establishment of
an operational system of psychiatric health care facilities by
distributing them according to the principles of their operation,
namely, hospitals for acute cases and society-based care facilities in
the residential area of their patients with definite role in provision
of work and training courses of working skills. A system of circulation
of persons with mental disturbances and psychiatric illnesses with an
aim of better integration of these people in the labour market will be
developed, too.
In the project, we will try to maximally use the existing resources of
psychiatric health care facilities regarding the employment of people
with mental disturbances and psychiatric illnesses in spite of the
differences and inconsequence in the previous approach to this issue, as
well as the uncertain situation concerning assessment, supervision
function, succession, validity of amount, duration and legal liabilities
of work, and the current offer of jobs and types of work.

Empowerment with participants in relation to the institutions involved
in the project will be implemented through equal involvement of every
institution who will perform equally significant work in the project.
Only the amount of work will differ, because the institutions differ
both in size and type of operation (hospitals for acute cases, hospitals
for chronic cases, hospitals which are oriented towards the residential
area and rehabilitation of their patients). The institutions will be
equally involved in preparation of activities, assessment of results and
formulation of political initiatives. Mutual supervision and control
will be ensured also in assessment of the obtained milestone results
both during the exchange-of-experience trip abroad, and during the visit
of foreign experts to Latvia.
The project activities – establishment of system of professional
evaluation of working skills, provision of training and maintenance of
working skills, as well as job opportunities for people with mental
disturbances and psychiatric skills – will provide this special group of
people with much better opportunities to compete in the labour market
and to receive better remuneration. This system will eliminate the
threat of discriminatory attitude in the labour market. Discrimination
in the labour market currently is directly related to the differences
between people with mental disturbances and psychiatric illnesses and
"regular" people. Mental disturbances and psychiatric illnesses deny
these people to "fit in" the society and aggravate the insularity and
sense of insecurity of these people; they feel uncomfortable and
different from others. Establishment of the system of employment that is
connected to the psychiatric health care service will facilitate gradual
and less shocking involvement of the target audience in the common
labour market, further the increase of working capacity of these people,
eliminate the deterioration of the whole functioning of the system in
relation the stressors in the labour market.

Basically, cooperation among applicants will be ensured though
assignment of identical work tasks for each member of the group of
applicant. This will motivate the applicants to consult among
themselves, analyse and operatively eliminate the possible errors,
coordinate their cooperation.
The cooperation and exchange of information among the applicants will be
ensured by holding regular meetings, by going on a business trip abroad,
by joint development of new policy aspects.
The applicants share several advantageous cooperation factors, namely,
they already work in the mental health care sector, they have acquired
experience of cooperation in assessment of the mental health service and
discussion of health care methods. The members of the applicant group
meet on a regular basis also outside of cooperation projects as they
gather for meetings at the state non-profit limited liability company
"Psihiatrijas centrs" SIA.
In the framework of this project, each institution will analyse the
situation surrounding the project target audience regarding the
opportunities of employment, availability of work and people's working
skills. Thus, through involvement of the target audience and joint
assessment of achievements together with cooperation partners, the
problems of every particular region and specific group of the target
audience will be identified. Each institution which participates in the
project will provide work for the target audience, thus directly
involving the target audience in implementation of this project. It will
also allow for a professional assessment and adjustment of working
methods and principles in every institution and every working place.
The project envisages regular visits of the project applicant to the
participants of the applicant group to directly share information of the
progress of the project, as well as assess the current achievements.

In 1824, the oldest psychiatric health care facility in the Baltic
states – "Aleksandra augstumi" was founded. Later on, the Riga
psychiatric hospital was created. In 2000, it was transformed into state
non-profit limited liability company "Psihiatrijas centrs". This
institution provides health care services, rehabilitation, social care,
organisational and methodical work. It is planned to transform the
"Psihiatrijas centrs" into the "Garīgās veselības valsts aģentūra ". The
experience in implementation of projects:
1. participation in the
project of the World Health Organisation "Monitoring of mental health
system and service", gathering of information;
2. participation
in the EU IMPHA project "European platform for facilitation of mental
health and prevention of mental illnesses". Planned costs: 4,500 euros;
funding of IMPHA: 2,250 euros;
3. implementation of the Latvian
Centre for Human Rights and Ethnic Studies (LCESC) and Soros foundation
projects "Improvement of ambulatory assistance centre, facilitation of
employment and support therapy for patients with mental illnesses in
Riga" – USD 3,306; "Establishment of support groups for persons with
suicidal tendencies after suicide attempt" – USD 11,997;
4. implementation of the project "Establishment of ergotherapy services"
in the framework of the Mentor programme. Mentor funding – LVL 2,191;
5. participation in educational project together with Oberfranken district,
Germany
6. participation in development, management, drafting of
tasks and assessment of performance of foreign and local consultants in
the subcomponent "Development of the mental health care system" of the
World Bank's reform of the health care system
