Activity details

To address the challenge of prevention and continuity of care (even during and after the COVID-19 outbreak) for the NCD patient community by implementing a "holistic" web platform. The platform will include institutional, validated, and accessible health information (including addressing the issues of chronic disease prevention, lack of adherence to therapies, integrated care, frailty, and lifestyle) from COVID-19 and NCD experts. Professionals with expertise in communication and social media management that could efficiently define the right channels for divulgation and set an appropriate tone of voice to make the information accessible to wider population. It will be published in different formats to increase accessibility and diffusion, to empower a greater range of individuals. More specifically the platform could provide: • Recommendation on preventive measures to minimize the risk of SARS-CoV-2 infection • Recommendation on preventive measures to minimize the risk of NCD-associated health problems • Information on NCD management to empower patients • Official health recommendations on COVID-19 from institutions • Relevant results from scientific publications, written in a simpler, lay-person format • Links to local services and support groups • Psychological support for the emergency (during and after) to both patients and caregivers • Videos from healthcare professionals on how to self-manage NCDs (symptom control and monitoring, diet etc) • Home exercise videos from a physiotherapist specialist in NCDs • Apps for symptom monitoring
