Activity details

Project CHANGE's main objective is facilitate the translation of clinical science to quality patient care. We aim to partner with the global healthcare community to improve patient outcomes in areas of Adherence to prescription, Integrated care and lifespan health promotion & prevention of age related frailty and disease through support of measurable learning and change strategies. The main goal is Improve the quality, safety and effectiveness of healthcare through facilitation and support of projects that increase the competence and performance of individual providers and healthcare teams, educate and empower their patients and implement practical solutions within real world care.
