The EU-funded United4Health project has published a report about how its 14 European project deployment sites have embedded telehealth technology into their care pathways. The project experts also give policy recommendations.

Using many visual elements, Telehealth in Practice - Care Delivery Models from 14 Regions in Europe describes how telehealth solutions improved the care management of chronic conditions such as COPD and diabetes at the pilot sites. Telehealth increased the levels of self-management of the patients and it enabled more timely care and support provided by care practitioners.

United4Health also published an Upscaling Telehealth Policy Brochure to help policy makers and to propose policy actions, since, according to Prof. George Crooks (Medical director NHS 24): "Evidence has shown that successful transformational change on a large scale, requires not only bottom up willingness to change, but also top down policy support."
