OBJECTIVE
AND CONTENTS OF THE PROJECT
There are large survival differences between American and European colorectal cancer patients. In the US 5-year relative survival for colorectal cancer was 58% (SEER program, white, 1983-85), while in Europe it was 45% for colon and 41% for rectum (EUROCARE, 1983-85). Differences were particularly evident among the oldest patients, aged 65 or more. This project's aim is to describe and interpret these survival differences in terms of patterns of care (stage, diagnostic determinants of stage and therapy) using the same statistical approaches and criteria for the selection of patients. Focusing on differences between age groups and comparing short term (6 months) and long-term (up to 10 years) survival, this study will help in understanding whether survival differences can be due to stage, treatment or to early diagnosis without later death (early detection bias). If these differences are not an artifact they can indicate the need of a major effort in order to improve the management of colorectal cancer patients in Europe. |

The aims of the project \"Impact of early detection on incidence rates of female genital cancers and the importance of estimating the prevalence of hysterectomies\" are the correction of incidence rates of uterine cancers by estimating the prevalence of hysterectomies in the population and the presentation and discussion of the results at a symposium in February which will bring together experts from Cancer Registries from all EC member states. The conventional method used to report age and site-specific cancer-incidence rates uses the total number of new cases during a given period of time as the numerator, and the total population \"at risk\" as the denominator. The population at risk is given as the annual average by age-group and sex for the period under consideration (1,2). However, consideration must be given to the numbers of women who have had a hysterectomy for a non-malignant condition, because such women are not at risk of developing cancer of the cervix or corpus uteri. Therefore, corpus and cervical canceer incidence rates may be biased, if hysterectomized women have not been removed from the population at risk. In Germany and in other European countries, incidence rates of cancer of the corpus and cervix uteri are not systematically corrected because the prevalence of hysterectomy is n ot known or considered to be too low to be significant. |

The aim of the project is to further investigate the causes of skin cancers (both melanoma and non-melanocytic skin tumours) and to elaborate a test to identify subjects at higher risk, test to be potentially used to refine the indications of primary prevention as well to define early diagnosis schemes. The results of the pilot phase, which was financed as a feasibility study by Europe Against Cancer in 1997-98, proved that the determination of the molecular type of melanin on hair samples offers new possibilities to define phenotypes at risk of developing melanoma and skin carcinomas. The pilot phase showed that the method is feasible on a population basis and that it can discriminate the phenotypes better than the previous techniques of visual examination could do. The present study aims to assess the relationship between the typoe of melanin (whether eu- or pheomelanin) and different types of skin cancer. The objectives will be attained through a case-control study, with three sets of cases (melanoma, Squamous- and Basal-cell carcinoma) and a set of controls recruited in 13 Centres in 8 European countries. Using such a population basis allows both to examine a wider range of skin phenotypes, and to realise a study of considerable size (over 1,000 case on total) within a short span of time. |

1. To estimate relative survival rates in ederly (>=65 yrs) cancer patients in the European countries by age classes, gender and calendar period. Analyses according to prognostic factors, such as tumour stage, histology and anatomical subsite, will be also performed only for those registries collecting data detailed and reliable enough for such a purpose. Particular attention will be paid to the oldest old (>= 90yrs), an age class often excluded from most of epidemiological studies. Both major cancer sites (all cancers combined, stomach, colon-rectum, lung, breast, cervix, corpus-uteri, ovary, bladder, prostate, non Hodgkin's lymphomas) and older age typical ones (e.g. multiple myeloma, lymphatic and myeloid leukaemias) will be selected for the analyses. 2. To relate these survival rates to availability and functioning of health care systems, welfare state (by means of social services), other socio-economic factors (e.g. educational level, financial resources by age, social environment) and demographic indexes. Such informations, regarding elderly populations across European countries, will be standardised according to the most common statistical indexes adopted by EUROSTAT and by National Statistical Institutes. They will be used to explain the observed differences in survival between younger and older patients and among different countries. |

OBJECTIVE AND CONTENTS OF THE PROJECT This project aims to improve the knowledge on cancer disease among the broad public and enables it to grasp scientific problems in an easy and simplified way. It also focuses on information on prevention issues and healthy behaviours to adopt and gives an overview of examples of good practice where treatments are carried out. Another important target of the project is represented by General Practitioners (GPs) seen as the interface between patients and specialists. In fact, GPs are among the firsts to share the problem with persons affected by cancer disease and they have the responsibility of helping them to choose the most suitable specialised centres. The project consists in a comprehensive Web-site promoted by the European Medical Association in co-operation with European Cancer Association, representing the needs of the network of patients, as well as with Hospital Cancer Institutes. The site is structured in three main parts : a multilanguage introduction on how to get the information and use the software tools, a second and third part with material developed explicity for the broad public and GPs. An innovative approach will be taken into account, which is represented by the following aspects : a) validation activity permanently carried out by a Scientific Committee formed by specialists appointed by Cancer Associations/Institutes. This will assure the high quality of the information put on the Web-site. The Committee will also improve the level of awareness among the broad public by drawing up specific reports (also published on the Web) which will give to people an effective tool to understand these specific topics; b) Multilanguage and multicultural sensitivity carried out through permanent assessment; c) A wide range of information designed for helping decision-making and improve critical knowledge on cancer related problems. This material will include : a database on the pathologies with clear explanations and ease of terminology to allow understanding among non-professionals, a code on preventional behaviours in different areas of pathologies as well as on healthy life-styles to adopt, a comprehensive and updated list on training courses for all pathologies concerned for medical doctors, a database of oncological centres as well as of the specialists working there, press review, information on the annual campaign within Europe Against Cancer Programme, relevant documents issued by the European institutions, a comprehensive list of links with cancer related Web-sites (to avoid overlapping of information), etc. |

The project's aims are : 1) to continue with the circulation of the two CD-ROM (Vizius Nikotina 2000 and Cancertrap) and to instigate further competitions both for the schools who have already participated and those who want to continue, 2) to supply and distribute throughout Europe two innovative tools which interconnects schools in Italy with the rest of Europe and permits the rapid exchange of data, experience and above all the supply of useful information to improve basic health education. Priority is given to risks factors of cancer, particularly tobacco, smoking prevention, nutrition and exposure to the sun. The feasibility phase consists of Workplace Community Analyses (WCA) in selected branches in Finland, Sweden, Germany and Spain. The bases for the activities are described int he document; the European Guidelines for Quality Assurance in Mammography Screening. (2nd Edition). The desirable endpoints for each member of this network is the establishment of a national breast screening service in which they operate as service and/or reference centre. Special attention is given to service, research and training of personnel involved in the screening process. OBJECTIVE AND CONTENTS OF THE PROJECT Meta-synthesis of the data of respective European colorectal cancer (CRC) screening trials (finished and ongoing) using the biomedical computer simulation model MISCAN. The aim is to further develop this evaluation tool for CRC screening in Europe and to obtain more knowledge of the natural history of colorectal cancer, the sensivity of the screening tests and the differences in these aspects between countries. This knowledge, together with the original data from the trials, will be used by the project-group (and be useful for others) to develop quality assurance guidelines for CRC screening in Europe, accounting for local differences. OBJECTIVE
AND CONTENTS OF THE PROJECT The project aims to train health care professionals (HCPs) in how to evaluate the Quality of Life (QL) of cancer patients. The expertise of the EORTC's QL Study Group (QLSG) and QL Unit will be used to design and evaluate a training course accommodating 20-40 persons. Participants will be able to identify when QL outcomes should be assessed and select appropriate QL measures. They will learn how to design and conduct QL studies, to obtain reliable, valid and clinically relevant information. The benefits and problems of cross cultural QL evaluation will be discussed. This proposal seeks support for course fees and accommodation for 20 HCPs from EU states to attend. Priority will be given to those active in oncology clinical trials, with little or no experience in QL evaluation, working in EORTC accredited centres for whom funding is not available elsewhere. |

AND CONTENTS OF THE PROJECT The three main aims of this project are : - to improve advanced training in genetic for health professionals (M.D.s and biologists) already working in the field of cancer prevention, a field of preventive medicine in which knowledge of Genetics is indispensable - to promote the establishment of a new professional figure for the genetic counselling of families and patients (genetic associates or genetic nurses) - to create an interface for communication with the general public, using specialized training in genetics for journalists and science writers. AND CONTENTS OF THE PROJECT It is planned to develop a practical instrument to measure the quality of life, including psychological and social aspects, of breast cancer patients. In so doing, specifically female experiences in various European countries and findings of research into women's health will be considered. This instrument is to take the form of an easily-interpreted profile for use in routine health care. It will enable both doctors providing treatment as well as other professional groups with no special training in psychodiagnostics to gain an overall view of the patient's individual situation and to detect any accomanying psychopathological symptoms at an early stage. A longer-term aim is to introduce a valid and practical procedure in oncological settings as a contribution to tertiary prevention and palliative care that will improve quality assurance in terms of systematic outcome management. |

AND CONTENTS OF THE PROJECT A. To describe the various palliative (hospital and home) care models for cancer which are being used in various countries of the European Union. B. To determine the quality of life of cancer patients who require palliative care in each country by means of the various instruments used currently in the EU for this purpose. This will be used as a basis for validating and adapting a Europe-wide questionnaire which will enable internationally comparable data on the European Union countries to be obtained in the future. C. To analyse and compare the impact on quality of life of the various types of palliative care for cancer, both in hospitals and at home. D. To analyse European vocational training programmes for palliative care for patients with tumours, identifying existing weaknesses and proposing an agreed curriculum for the undergraduate and post-graduate training of social/health professionals based on the experience acquired and results obtained under aims A and B. E. To prepare the ground for a European network of researchers and professionals who are engaged in palliative care for patients suffering from tumours with the aim of promoting a permanent forum for communications, exchange and information between groups and for bibliographical material which will help form a fuller picture of this subject. |

AND CONTENTS OF THE PROJECT The project aims at contributing to the implementation of action 17 and 18 of the Europe Against Cancer Programme of the Third Action Plan (Officiel Journal of the European Communities, 10/04/96, N° L 95/15), namely to improve the theoretical and practical knowledge of cancer of the European health professionals, to support their mobility among the Member States, to increase their teaching skills and the consequent transfer to knowledge and information to their colleagues at national levels, and to offer the possibility to attend ESO courses to 40 doctors from all Member States of the European Union. AND CONTENTS OF THE PROJECT This project aims at further implementing the feedback process on the content of START (State of the Art Oncology in Europe). In the light of the preliminary results of the previous pilot project. It is felt taht the feedback process should be mainly based on the European cancer societies. This will allow to extend the feedback mechanism to the full text of each chapter (the pilot project focused only on selected statements of the chapters), and, in a formal fashion, to all the codified statements which will be felt to be most questionable and/or relevant (in the pilot project this selection had been made by the START editorial board). In addition, this new project will cover all the chapters available on the Internet by the end of 1998. Thus the aim of this new project is to verify the feasibility of a more comprehensive feedback mechanism on the START chapters, which could constitute the definitive pattern for the START feedback process from the year 2000 onwards. A permanent feedback process is felt to be a major requisite for START to be a true state-of-the-art European instrument. |

AND CONTENTS OF THE PROJECT The aim is to reduce the morbidity after radiotherapy but at the same time preserving or increasing the probability of local control of the tumour. Most radiotherapy related morbidity is caused by unintended delivery of dose to healthy tissues above the known levels of tolerance. The prescription itself as well as any process or tool involved in the preparation and delivery of raidotherapy may carry (hidden) elements that may lead to exceeding tolerance doses. The first task is to improve dosimetry as small deviations in the dose might result in adverse effects. The methodology developed in the EC network project will be followed. The second task is to assess the present situation regarding morbidity in order to be able to estimate the efficiency of the various treatment regimes. Combination of task 1 with task 2 allows to exclude significant dose deviations as a possible cause of complications. |
