Role of the duodenum in the regulation of food-stimulated incretin and insulin secretion.

Incretins released by the gut (GIP and GLP-1) are key mediators of food-stimulated, glucose-dependent insulin secretion. GLP-1 may play a role in the regulation of the pancreatic alpha and beta cell function. We aim to assess the role of the duodenum in the regulation of pancreatic alpha and beta cell function. Patients scheduled for duodeno-pancreatectomy will undergo a mixed-meal tolerance test in conjunction with the infusion of stable isotope tracers (glucose) during which insulin, c-peptide, GIP and GLP-1 concentrations will be measured, before and after surgery. The post-surgery follow-up will allow us to assess the effect of the removal of the duodenum on incretin and insulin secretion in response to food ingestion
