Intraoperative Neuromonitoring during open-heart surgery

Recent studies on epilepsy animal models and on brain slices demonstrated a clear-cut contribution of inflammatory mediators to neuronal hyper-excitability and seizure induction. Open-heart surgery, and particularly external body circulation - during which blood cells impact on non-biological tissues, releasing pro-inflammatory compounds - is considered the in vivo model applicable to humans. Electroencephalographic recordings permit the identification of possible paroxysmal discharges in the central nervous system (CNS) and its concomitant registration during this type of surgery would permit real-time identification of irritative phenomena. Whenever feasible, a temporal correlation of such discharges with the plasmatic concentration of inflammatory cytokines and other inflammatory substances will be determined, in order to define a relation between the general state of inflammation and CNS responses.
