A combined biochemical and cell biology apporach to improve the pharmacological treatment of PH1: from pyridoxine derivatives to proteostasis regulators.

The mechanisms by which B6 reduces the synthesis and excretion of urinary oxalate are not well known. Our preliminary studies suggest that vitamin B6 can affect AGT in many ways including increasing its activity, stability and modyfying where it is targeted within the cell. A better knowledge og how vitamin B6 works in PH1 would allow us to design modified forms of vitamin B6, which would be more effective in helping AGT attain a functional structure and enhance the specific effect of vitamin B6 by combining it with small molecules known to have a non-specific effect on protein folding. This project would allow us to define criteria correlating the mutation, type of enzyme dysfunction and B6-responsiveness. Such work would be a pre-requisite for a patient-tailored therapeutic management of PH1 but could also lead to novel strategies in terms of treatment.
