Suffocation

The product poses a risk of suffocation because eight satin loops have a circumference of more than the maximum limit of 360 mm laid down in the standard EN 1466. The average circumference of each of the eight loops is 570 mm.

A 9-month-old child died following strangulation with a loop into which the child put its head.

Electric shock

This product poses a risk of electric shock because:

Burns

- The packaging does not provide any information about the amount of gel to be used, where to use it (open or closed space) or the conditions of use (for lighting or relighting fires). It only says "Use in oil lamps: Filling height – do not fill the burner to the top".

- This preparation is made up of methyl ethyl ketone (5%, (Flashpoint: -4 °C) and ethanol (75-95%) (Flashpoint: 13 °C). Its composition means that the AGA burning gel is a dangerous preparation and, according to existing legislation, is classified as "Highly flammable". As such, it must be stored, packaged and labelled in accordance with Decree-Law No 82/2003 of 23 April 2003. It has a very low flashpoint and is therefore likely to present serious dangers to the user when used near a flame, as is the case under its normal conditions of use. The fact that the product is a gel and could therefore be spilt increases the danger surrounding its use, due to its flammability and its toxicity.

- Although the warning information on the product’s label states "Keep away from sources of ignition", the fact that the purpose of the gel is to facilitate burning means that it may be used to relight a fire and the user might consequently get burnt.

- After various tests to open and close the bottle, it was concluded that the safety cap does not always work, which poses an additional risk when using this product.

- Although the label carries the symbol for danger (F) printed in black on an orange background, there is no tactile warning of danger.

- The warning information on the label is difficult to read.

Injuries

The product poses a risk of a person and, in particular, a child, falling out of a restricted but open window. The restricted window hinge should restrict opening the window to approximately 100mm and the designed resistance load to overcome the restriction should be at least 600N. The potential is for the restrictor mechanism to fail somewhere below 600N of pressure and the window sash to open further than 100mm leading to a risk of falling out of the window. During testing, the restrictive hinge gave way on some occasions between 600N and 150N.

This product does not comply with the relevant standard BS 6375 and BS 8213.

No accidents have been reported but there has been one complaint of failed product.

Fire

In some units, the fuel tank vent hose may have been kinked or bent during set-up or service, creating the possibility of a fuel leak into the engine compartment. In certain models equipping the electric fuel pump, the kinked fuel tank vent hose could cause the fuel pump to separate from its mounting flange. Refuelling the vessel after this occurrence could result in fuel leaking into the engine compartment leading to a risk of fire.

This product poses a risk of choking because a squeaky mechanism may be detached from the toy and may be put into mouth by children. The squeaky mechanism has very small dimensions and during the laboratory tests it fitted within the small parts cylinder. This product also poses a chemical risk because these toys contain phthalate DEHP, which is classified as category 2 – toxic material for reproduction (dolphin - 5, 61 %, duck - 14, 3 %, frog - 17, 0 %).

This product does not comply with the Toys Directive and the Toys Standards.

Injuries

The front master brake cylinder assembly installed on these vehicles had been built with inadequate lubrication on the secondary cup.

This could cause too slow return of the piston upon release of the brake and in extreme cases, the piston may not return completely. If this happens, the rear brake light could remain illuminated which could result in an accident, since the rider cannot warn the drivers of other vehicles when reduces the speed of his motorcycle.
