Poisoning

The sample of that product was submitted for testing and found to fail the following requirements of the standard:

– wick guard not fitted, wick exposed;

– filler cap not secure, a push fit glass tube;

– oil leaked freely when lamp tipped over;

– no labelling, including nothing about the danger of oil ingestion and what to do if it is accidentally ingested.

This product poses a risk of poisoning that can occur following the ingestion of even small quantities of the oil used as fuel. A condition known as lipoid pneumonia may be caused by the entry into a child’s lung by a very small quantity of the oil, as is likely in the event of aspiration following vomiting. Part of the lung may fill with fluid which may then become infected or turn to fibrous tissue. The short term effect is serious breathing difficulty that may result in death. The hazard recognition of this possibility is remote unless clear instructions and warnings are used.

The risks associated with fire are obvious, and hazard recognition is good.

This product does not comply with the relevant European Standard.

During the combustion of the candle, the coffee beans incorporated in the candle caught fire, with a flame particularly powerful (reached a height of approximately 10-12cm), resulting in the candle cut in half and wax overflowing at the sides. Judging from the flame intensity, it is likely that burns may be caused in individuals that have not realised that the candle is being destroyed.

Under certain conditions, a fire can be generated in closed spaces, due to the powerful flame.

Injuries

There is a possibility of an accident due to lose of control of vehicle.

On certain Avensis vehicles with 1.6 and 1.8l petrol engine manufactured in United Kingdom, and Corolla and Prius vehicles manufactured in Japan, which are all equipped with Electric Motor assisted Power Steering [EMPS], there might be a possibility that connections may become lost due to insufficient strength at the intermediate shaft and sliding yoke, which connect the steering wheel to the steering gearbox assembly. This might happen during conditions such as when the steering wheel is turned repeatedly strongly to the full lock position (max. steering angle), right or left, or where the tires repeatedly strongly hit the curb when manoeuvring.

Due to various safety hazards presented by the product, it is extremely dangerous to operate and is likely to cause injury to the rider and to any other persons in the vicinity of the machine.

In particular, it poses a risk of catching body parts/clothing in moving chain/sprocket and burns from exhaust because:

-the drive chain and sprocket are not sufficiently guarded;

-the wiring harness/electrical connectors are insecure and chafing on various parts of the machine;

-the exhaust is insufficient in size to carry out its purpose;

-there is no speed-limiting device;

- the frame welding is poor and holed.

On the basis of a functionality test (in shoes, socks and bare feet), the individual layers of the mat slide on each other, causing the upper foil to twist, which represents an undesired slip. Considering the character and design of the product and the age limit for its use (>12 months), the risk of injury as a result of a slip and subsequent fall can not be excluded while using the product.
