Electric shock

The notified product poses a risk of electric shock when it is being cleaned. (The preconditions for fatal electric shock are: 1. The aquarium light is plugged into the mains and the two-pole cord switch is in the on position. 2. The aquarium cover falls into the water during cleaning and the user reaches into the water).The appliance failed the test for water ingress. In addition, warnings and indications required by the standard are missing, cross-sectional area of the supply cord and the cord grip are inadequate. The notified product does not comply with the LVD Directive and the relevant European Standards.One incident reported:

According to an anonymous report from a consumer, the aquarium cover with the lighting unit fell into the water during maintenance work. This gave rise to a minor electric shock. When the cover was later retrieved, the neon tube holder had become detached.

Voluntary measures: Voluntary withdrawal from sale and recall from consumers by manufacturer.

The product does not comply with the relevant European standard EN 1335 (product failed the front stability and forward overbalancing tests). One injury concerning this product has been reported.

Injuries

This machine is extremely dangerous to operate and is likely to cause injury to rider and to any other persons in the vicinity of the Machine.

Vapours from fuel tank could affect riders' control of machine and also cause potential fire hazard (the fuel tank is vented via the fuel cap, directly in front of the rider).

Catching body parts/clothing in moving chain/sprocket may happen (the machine’s drive chain and rear sprocket is almost completely unguarded).

Voluntary measures: Voluntary withdrawal and recall of the product by retailer.

Choking,
Strangulation

The toy may present risk of choking because it contains small parts which fit to the small parts cylinder. The cord that is meant for fixing the toy to the pram may present risk of strangulation because the cord has more than 750mm while extended with force 25±2N. The following warning is missing: "Warning! To prevent possible injury by entanglement, remove this toy when the child begins to push up on hands and knees".

Compulsory measures: Withdrawal from the market was ordered by the authority. In addition, the importer has been obliged to inform consumers about the risks.

Risk of choking and chemical risk. During testing, squeaking mechanism, tongue and front legs became detached. Small parts constitute a choking hazard for children under three. In addition, product may release phthalates that are prohibited in toys under the German law and the pending European Directive. Phthalate (DEPH) content 25 % in plastic. The product does not comply with the Toy Directive and the relevant European Standard.

Voluntary measures: Voluntary withdrawal and destruction of the products by the supplier.

Injuries

The hatchet presents risk of injury because:

1) The handle is not mechanically fastened to the head, but is attached using a red binding mass. In one case, the handle was attached to the head by a yellow synthetic resin adhesive. The head fell off the handle because of its own weight, and it could easily be detached by hand.

2) The hatchet head was made of an unsuitable material. This was not the prescribed forged steel, but a brittle coarse-grained cast material with many defects (flaws and cavities), which weaken it even more. When the hatchet is used the head can shatter or split, and flying fragments can cause serious injuries.

Voluntary measures: Voluntary withdrawal from the sale by retailer.

Injuries

The hammer presents risk of injury because:

1) During the tests, the handle of the hammer broke directly beneath the head after three blows. An unsuitable type of wood was used. The head of the hammer flew off.

2) The head is made of cast iron rather than the required forged tool steel. Cast iron is coarse-grained and brittle and therefore totally unsuitable for tools, especially hammers. Defects in the material (flaws and cavities) weaken it even more. When the hammer is used the head can shatter or break, and flying fragments can cause irreversible injuries.

Voluntary measures: Voluntary suspension of sale by retailer.

Electric shock

Product poses a risk of electric shock since basic insulated wires are in contact with accessible metal parts.

With regard to wiring and constructional requirements, there is no cord anchorage nor bushing and the plug is not of IEC60083 type. Moreover, wire ways contain sharp and burred metal parts and the connection of the lampholder made of screw is not protected against loosening. Product does not comply with the LVD and the relevant European standard EN 60598.

Compulsory measures: Compulsory recall and fine imposed by the authority.
